Ischia sits at the northern end of the Bay of Naples, about an hour by ferry from the mainland. The island runs about 10 km across, so reaching the best beaches and thermal parks on the western and southern sides requires a bus or taxi. That said, Ischia Porto and the connected Ischia Ponte area offer enough — a castle, decent restaurants, and pleasant lanes — to fill a half-day without ever leaving the immediate area.

Ischia is genuinely volcanic. Thermal springs bubble up across the island, and the thermal park experience here is more legitimate than in many Mediterranean spa destinations. If that is your main interest, plan your day around it; the parks take 2-3 hours minimum and are worth the entrance fee. If you just want a beach and some Italian atmosphere, the island delivers that too without much effort.
